ABSTRACT

OBJECTIVES: To evaluate and compare the magnitude and distribution of stresses generated on implants, abutments and first molar metal-ceramic crowns using finite element analysis. METHODS: Preliminary three-dimensional models were created using the computer-aided design software SolidWorks. Stress and strain values were observed for two distinct virtual models: model 1 - Morse taper and solid abutment; model 2 - Morse taper and abutment with screw. A load (250 N) was applied to a single point of the occlusal surface at 15° to the implant long axis. Von Mises stresses were recorded for both groups at four main points: 1) abutment-retaining screws; 2) abutment neck; 3) cervical bone area; 4) implant neck. RESULTS AND CONCLUSION: Model 1 showed a higher stress value (1477.5 MPa) at the abutment-retaining screw area than the stresses found in model 2 (1091.1 MPa for the same area). The cervical bone strain values did not exceed 105 µm for either model.

ABSTRACT

Introdução: o presente trabalho aborda o tema "O papel da atenção básica no Sistema Único de Saúde". A qualidade dos serviços de saúde remete à necessidade da adoção de um modelo de organização em saúde, como forma de otimizar os serviços de saúde pública, tornando-os mais eficientes. Objetivo: o objetivo deste trabalho é realizar uma revisão da literatura sobre os desafios da atenção primária e a implementação da Estratégia Saúde da Família no Brasil. Métodos: o estudo foi realizado a partir de pesquisa bibliográfica, que permitiu abordar o tema, fundamentando-o em fontes como livros, periódicos, artigos publicados e consulta a meios eletrônicos. Resultados e conclusão: concluiu-se que oferecer atenção à saúde frente às inovações da tecnologia não basta. É preciso também introduzir um processo racional e, sobretudo, bom senso no desempenho diário das equipes de saúde e da gestão.


Introduction: the current study approaches the role of primary care in the "Unified Health System". The quality of health services refers to the necessity of adopting a model of organization in health, in the order to optimize the public health service, making them more efficient. Objective: the objective is to review the literature on the challenges of primary care and the implementation of the Family Health Program in Brazil. Methods: the methodology used was carried out through a literature review, which allowed to analyze the issue, based on sources as books, periodicals, published articles and consulting to electronic media. Results and Conclusion: it was concluded that providing a health care considering the innovations of technology is not enough. It must also be introduced a rational process, and especially, good sense in the daily performance of the health staff and management.

ABSTRACT

Frente a crescente utilização de implantes osseointegrados nas reabilitações orais, as dificuldades foram surgindo junto a necessidade de reabilitar arcos dentários com níveis avançados de comprometimento dos rebordos alveolares remanescentes. Assim, os enxertos ósseos passaram a ser uma opção no sentido de possibilitar e/ou até mesmo melhorar os resultados finais da prótese sobre implante sobre tudo em maxila. Os autores relatam o caso clínico de uma paciente de 40 anos de idade, portadora de prótese total superior que desejava substituí-la por implantes. Detectado a avançada reabsorção óssea da maxila planejou-se cirurgia de enxerto autógeno da crista ilíaca. A técnica usada foi do tipo “onlay”, e simultâneo levantamento e preenchimento bilateral do seio maxilar. Conclui-se que a reconstrução de maxilas extremamente atróficas com o uso de protocolos de enxertos de osso autógeno oriundo da crista ilíaca podem devolver o volume em espessura do osso maxilar viabilizando a fixação futura de implantes e facilitando o sucesso das próteses


Due to the increasing use of osseointegrated implants in oral rehabilitations, difficulties have been arisen with the need of rehabilitating dental arches with advanced levels of remaining alveolar ridge involvement. Thus, bone grafts have become an option in the sense of both making possible and improving the results of prostheses over implants mainly in the maxilla. The authors report a clinical case of a 40-year-old patient who desired to substitute an upper total prosthesis for implants. The autogenous iliac crest graft has been chosen due to the advanced bone resorption the maxilla. The onlay technique has been used with simultaneous lifting and bilateral filling of the maxillary sinus. It has been concluded the reconstruction of extremely atrophic maxillas with autogenous iliac crest grafts may restore the maxillary bone thickness making possible the future placement of implants for better prosthetic results
